Locke Hill Elementary Kindergarten Round-Up

If you have a child who will be five years of age on or before September 1, 2006, you are encouraged to attend. Registration will be held in the school cafeteria. Parents may bring their future Kindergartners with them to the registration. Kindergarten teachers will be available to give tours of the Kindergarten classrooms and the school.


To register a child for Kindergarten you will need to bring:
1. The child’s official birth certificate
(not a certificate issued by the hospital)
2. The child’s social security card.
3. Your proof of residence (ie. Utility bill)
4. Parent’s picture ID
5. The child’s up-to-date shot record including the following immunizations:
DTP / DTaP - 5 doses (or 4 doses if one dose is on or after the 4th birthday)
Polio - 4 doses (or 3 doses if one dose is on or after the 4th birthday)
Measles – 2 doses
(1st dose must be on or after 1st birthday; 2nd dose may be recorded as part of MMR)
Rubella – 1 dose
Mumps – 1 dose
Hepatitis B - 3 doses
Hepatitis A - 2 doses
(2nd dose must be given 6 months after the first dose)
Varicella – 1 dose on or after 1st birthday
(or verification that the student has had chicken pox)

All immunizations need to be up-to-date in order for the school to accept the registration packet.


Registration for kindergarten is on a first come, first served basis. If you have any questions regarding kindergarten registration please call Locke Hill Elementary at 397-1600.
