Part II: Examples and Guidelines for Use of the Forms.com Tools

The District has purchased a one-year subscription (October 2002 - October 2003) to Forms.com for every campus webmaster. With this unique service, you can easily add "interactive databases" to your site!

Imagine the possibilities!
parent surveys, order forms, guest books, registration forms, online assessments, questionnaires, lesson plan collections, online logs, reservation forms, alumni postings, community feedback & opinion polls, voting & mock elections, and more!

The course enrollment form that you completed in Part I is a perfect example of seamless data collection! YOU have absolute control over the appearance of your forms, because YOU create them in Dreamweaver and add them to your site. Data submitted to your website is "hosted" on the Forms.com server in Atlanta, Georgia. Using your unique webmaster login and password, you can:


A FEW SIMPLE RULES
  1. For privacy & security reasons, do NOT collect personal or sensitive data (social security numbers, ID numbers, grades, credit card numbers, etc.)
  2. Be especially cautious when collecting data from students. Generally, student surveys are not recommended.
  3. Get your principal's permission before posting any type of form.
