| DTP/Td/DT/DtaP- 4 doses by 4th birthday (The 4th dose must be on or after the 4th birthday.) Polio- 3 doses by 4th birthday (The 3rd dose must be on or after the 4th birthday.) Measles- 2nd dose by age five (The 1st dose must be on or after the 1st birthday.) Mumps (Must be on or after the 1st birthday.) Rubella (Must be on or after the 1st birthday.) Hib not required after five years of age. Hep B- 3 doses Hep A- 2 doses to be completed by August 1, 2009 (2nd dose must be given 6 months after the first dose.) 2 doses Varicella (Chicken Pox) or verification of illness |
