Reimbursement Form.  This form is to be completed for any check disbursements from the OHT PTA.   All reimbursement requests should include original receipts.  Please make a copy for your records. 

Membership Form.   Print a copy of this form, complete it and send it into school with your child.  Please include $4 for each parent/guardian you wish to sign up as a member of OHT PTA.
