DPT/DTap/Td/DT
 5 doses, unless fourth dose was administered on or after 4th birthday.
 Polio (OPV) 
 4 doses, unless third dose was administered on or after the 4th birthday.
Measles 
 2 doses by kindergarten entry
Rubella/Mumps
 1 dose by kindergarten entry
Hepatitis A 
 2 doses for 1st graders, Kinder, Pre-K, ECC, and PPCD
Hib
 One dose.  A series of three if given in infancy.  Required through 4 years of age.
Hepatitis B 
 3 doses required for K-11 
Varicella (Chicken Pox) 2 doses for 1st graders, Kinder, Pre-K, ECC, and PPCD
Tetanus (Td) 1 dose required within last 10 years for the last DTP/DTaP dose.
PCV (Prevnar) Please have the School Nurse evaluate PCV immunizations.

Families who have private insurance are requested to receive 
vaccines at their private physicians or HMO's. 
Those who have no insurance and wish to use the Health Department may 
call 210-207-8750 to find the most convenient clinic site. 
Families on Medicaid are requested to bring their Medicaid card or 
number to the site where vaccines will be administered.
