Student COVID Data Form
Black Knight Families,
A Google Form has been created for parents and guardians to input information regarding student COVID related absences so you do not need to contact the campus directly. With this information, the campus will have the data needed for the nurse to document each case and for the student to be placed in the system for Remote Conferencing. If more information is needed, the campus nurse or administration will reach out to you.
Attached are the guidelines to follow for COVID related scenarios that you or your student may encounter. If you have any questions or concerns, please contact the campus. Of course, if you need more guidance or assistance please reach out to the campus.
