New Employees who have attended the New Employee Orientation were provided materials concerning the various health and supplemental insurance plans offered through employment with NISD. Enrollment forms for New Plan Year 2010 were also provided during the orientations. These forms were to be returned to the Benefits and Risk Management Office as soon as possible, but not later than 30 days after the employee’s date of hire. The forms may be returned by mail to the Benefits and Risk Management Office at 5617 Grissom Road, San Antonio, TX 78238; faxed to (210) 257-1182 or emailed to Mark.Hardison@nisd.net.
Changes to Current or 2010 Insurance Plans
Elections made during the enrollment period are fixed and cannot be changed throughout the year except within 30 days of a qualifying life event (such as divorce, birth, death, etc.) For a complete list of qualifying life events and the allowable changes please click here. During the period December 14, 2009 through January 5, 2010, any request for a change in insurance coverage due to a qualifying life event, must be submitted on a Qualifying Life Event Worksheet.
