Students and Parents,
If your son or daughter will be trying out for sports and/or enrolled in the Athletic Period for the
2023-2024 school year, please review and complete the following requirements.
REQUIRED: All incoming 7th graders and any 8th grader entering Athletics for the first time will
need a Sports Physical, Medical History Form and the completion of Rank One Forms.
All incoming 8th graders who are currently in the Athletic period will also need to complete the Rank
One Forms and Medical History Form.
If your son or daughter has been diagnosed with Asthma, the Asthma form will need to be completed
by a physician as well. Please print the attachments and complete the necessary paperwork. The
sooner we get the paperwork, the sooner we can clear your child for the program.
Rank One online forms and Medical History Forms. You may purchase Athletic Uniforms during
class time.
REQUIRED: Our Athletic department will use a communication tool called Sports You.
Please Download SPORTSYOU app. We will be posting relevant information regarding our program,
including information regarding all tryouts and upcoming events. We encourage both parents and
athletes to join the group. Those familiar with the site can register using the following team

Attached is more information about the site and how to join our Team for our new users. Further
groups will be created that will be sport-specific. More information on that will be posted on the
general site for the corresponding Boys and Girls groups.

before tryouts.
