Documents Required for Registration:
Documents may be uploaded into the online system or brought to the school for copies to be made
- Child's Social Security Card (Optional)
- An original certified copy of the child's birth certificate
- An up-to-date immunization record
- Proof of residency:
- a current or prior month's utility bill; or
- a sales, lease or rental contract; or
- a notarized Affidavit of Residence (AOR) form
