Parent/Medical Notes must be turned in within 3 days of absence and student's ID number written on the excuse along with periods missed.
All notes can be brought to the attendance office or emailed directly.

Any forgotten materials may be delivered to the Attendance Office to be picked up by the student. If possible parents should notify the student that the materials are in the office and they can come between classes or during lunch to get the items.
CHECK OUT PROCESS FOR STUDENTS WHO DRIVE TO CAMPUS
- All students who drive to campus, including students 18 years of age must have written permission from parent/guardian to sign themselves out of school early for appointments.
- Parents/Guardians MUST email request to the attendance office first thing the morning of the appointment.
- Dismissal slip will be delivered to studentsโ class.
- Student must sign out through the attendance office when leaving campus.
- Parent/Student will need to follow this process each time a student that drives to campus has to leave.
CHECK OUT PROCESS FOR STUDENTS THAT DO NOT DRIVE TO CAMPUS
- All students who do not drive to campus must have a parent/guardian/emergency contact come to the attendance office with a photo ID to check the student out,
- Student will be called from class upon parent/guardian verification
- Please allow enough time for this process.

If you need to withdraw from O'Connor you will need to speak with Mrs. Castillo or Ms. Martinez in the Attendance Office A108.
For a change of address please email proof of residency along with a copy of parent/guardian photo ID to oconnorattendance@nisd.net
- Acceptable forms for Proof of Residency: CPS, SAWS or a trash bill (dated within the past 30 days)
- Bill must be first full-page showing usage at the new address
Students with absences due to certain medical
conditions with doctor's documentation may
qualify for a medical waiver. Please contact our attendance secretaries for more information.

The Campus Parking Application can be submitted online, once completed permits will be available to pick up in A104.
If you need to turn in items found on campus or retrieve items lost on campus come to A104. Mrs. Swank or a secretary can assist you.
Student ID Cards will be made in the Library before school, during lunches and after school.
Juniors and Seniors may go off campus for lunch if they have a parent permission form on file and are in good standing for grades and attendance. Please have a student ID and parent permission form completed, stickers may be picked up in the VPO office.
(Verification of Enrollment) is obtained through the APO's Office: students need to sign up in person or email the office at least 2 business days in advance and the VOE will be available to pick up in the APO's Office A104.

VOE's are only good for 30 days, so please make sure you have an appointment before requesting.
